Dr. Peratt does not accept insurance or work with a billing service
Payment is due in full at the time of service
Cash, check, debit cards and credit cards are accepted
Dr. Peratt is considered an "out-of-network" physician for PPO insurance plans. Certain insurance plans provide substantial benefits for "out-of-network" services. Dr. Peratt can provide you with a "super bill" (a detailed receipt) which you can submit to your insurance company
